For childcare charges to be reimbursed by the UO Childcare Subsidy Program, your care provider must either be registered/certified in the state where the dependent is receiving care, or your care provider must pass a background check and be approved by UO Childcare Subsidy staff.
For care providers who are not registered/certified in the state where care will be provided, you will need to submit background check documentation. UO Childcare Subsidy staff will accept background checks conducted by any legitimate agency or through any legitimate registry. Participants commonly use the process provided by the Criminal Justice Information Services (CJIS) Division of the Oregon State Police. This process is detailed below.
Background check arrangements are the responsibility of the program participant. Additionally, program applicants or their provider must pay any upfront costs associated with completing a background check. Applicants may apply for the Care-Finding Fund to be reimbursed, subject to limitations.

Criminal History Record Check Process: Oregon State Police

- Complete the form
- Requests must include:
- Individual’s full name (provider who you are checking)
- Date of birth
- Social Security number (if available)
- Current or last known mailing address
- The request cannot be processed without a current or last-known mailing address.
- Name and mailing address of the person or agency making the request (in this case, you, the student parent)
- Fee of $33 per request
- The OSP accepts checks or money orders through the mail, payable to Oregon State Police.
- You can also pay with Visa or MasterCard debit or credit card in person at the Salem General Headquarters, and a photo ID is required.
- Incomplete or illegible requests may delay or prohibit processing.
- Be sure to include your own contact information so that results are sent to you.

If the check finds a record, the person on whom the check is being conducted will have fourteen days to challenge the accuracy of the criminal offender information before it is disclosed to you.

- You should receive the results in letter/report form from the Oregon State Police through the mail. To receive your childcare reimbursement, you will need to provide a copy or scan of the letter/report that you receive from the Oregon State Police to the subsidy staff. You can attach this to your subsidy application or attach it to a Childcare Subsidy Change Form.
